Published fees and policies are intended to facilitate our work together and to clarify our mutual expectations.
Fee Schedule

- Individual counseling, psychotherapy for adults and young adults (from age 18):
- 50-minute session (office or telehealth)

- Home visits (by special arrangement): $340
- Saturday sessions (by special arrangement): Fee+40%
- Intake session only: +$25 administrative charge and new account setup (office and telehealth)
- Couples counseling, premarital counseling, marriage therapy, discernment counseling
- 50-minute session (office or telehealth)
- Payment by cash, check, credit card (preferred)
- $170/session
- Home visits (by special arrangement): $340
- Saturday sessions (by special arrangement): Fee+40%
- Intake session only: +$25 administrative charge and new account setup (office and telehealth)
- Families (3+ attendees) conjoint counseling with adult and/or child family members
- 50-minute session (office or telehealth – 100 minutes recommended)
- Payment by cash, check, credit card (preferred)
- $200/session (office or telehealth)
- Saturday sessions (by special arrangement): Fee+40%

- Trauma Treatment for emotional or physical trauma, delayed physical/sexual abuse recovery, and PTSD (with EMDR, CBT, CPT)
- 50-minute session (office only, telehealth not available)

- Veterans, Reserve & Active Duty (US services only)

- For individual veterans and their spouses
- Services provided by Dr. Z to qualifying veterans, reserve & active duty US military under Headstrong sponsorship
- You must be signed up with Headstrong and accepted into the program
- Police, Fire, First Responders
- 50-minute session (office or telehealth)
- For all mental health and counseling issues
- For individuals and their spouses
- Services provided by Dr. Z to qualifying police, fire, first responders under their department sponsorship
LEGAL MATTERS
I do NOT routinely testify on behalf of clients in divorce or other civil causes. However, I will fully cooperate with your attorney to provide any documentation that may be necessary. On a case-by-case basis, I may be available to serve as an expert witness. My fee for this service is $1,850/day.
LETTERS, TAX DOCUMENTS, INSURANCE/LEGAL SUMMARIES & EVALUATIONS
The fee is $170/hour of preparation per document.
CAREER & PERSONALITY ASSESSMENTS
Myers-Briggs MBTI® Includes interpretation, report, and one session: $360
Strong Interest Inventory (SSI) Profile Report® and interpretation: $270
MBTI+SSI Package, with interpretation and report: $504 (20% off)
Memory and Cognition (MOCA) Test and interpretation: at the office $170, home visit $340
Practice Policies
TELEPHONE CALLS
You’re welcome to call Dr. Z if you have an urgent matter to discuss in between sessions. His schedule permitting, Dr. Z will take your call. Your credit card will be charged a prorated charge for calls that are longer than 10 minutes.
TELEPHONE CALLS BY THIRD PARTIES
Due to ethical, confidentiality, and legal privilege constraints, Dr. Z will NOT take a phone call from attorneys, ex or divorcing spouses, relatives, friends, or other service providers, unless previously authorized in writing by the client of record.
INSURANCE
Dr. Z does NOT take insurance of any kind. He is an out-of-network provider. Upon request, a “superbill” can be provided after each session, which you can use to file for out-of-network insurance reimbursement.
DISABILITY AND FMLA CLAIMS
Dr. Z can fill out the paperwork required to support a disability or FMLA claim for his client of record only, provided that the client is current with payments and is in psychotherapy or counseling with Dr. Z at the time of the request.
DISCHARGE FROM HOSPITAL
If you or your relative is discharged from the hospital and require follow-up psychotherapy or counseling, please give the hospital Dr. Z’s contact information where they may send your discharge papers. Call our office if you need additional details.
PAYMENT
Payment is required at the time of service. Forms of payment accepted: cash, all major credit cards, debit cards, HSA cards. A credit or debit card on file is required of all clients and will be collected at the first session.
NEW ACCOUNTS
A one-time account administration fee of $25 is applied to all new clients, payable with the first session.
CANCELLATIONS AND NO SHOWS
A professional time commitment is set aside and held exclusively for you. If you need to cancel or reschedule an appointment, please notify our office at (678) 554-5632 at least 24 hours in advance. If the change is made with less than 24 hours’ advance notice or if you do not keep your scheduled appointment and cannot provide a medical reason, you will be responsible for the full fee. The cancellation fee may be waived if you reschedule within the same week.
OFFICE AVAILABILITY
Our voice mail at (678) 554-5632 is monitored during business hours. If possible, your call will be returned the same day, or the next business day at the latest.
EMAIL COMMUNICATIONS
To fully protect your confidentiality and privacy, you are advised to limit your use of email to appointment requests and appointment management only.
MENTAL HEALTH EMERGENCIES
If you feel that you and/or your relative are in immediate danger or that you or your relative are a danger to self or others, please dial 911 or contact a local psychiatric hospital, or go to the nearest hospital emergency room. Our office does not provide emergency mental health services.
REFERRALS
We do not make referrals for certain services such as legal representation, financial advising, and other non-clinical providers. Dr. Z may be able to refer a psychiatrist or medical doctor with whom he may have a professional acquaintance, but he is unable to guarantee availability, response time, or fees that meet your requirements. Dr. Z’s recommendation is that you search for a clinical provider on your insurance provider’s portal or an online directory such as Psychology Today.
